N.O.W. Profile Privacy Policy

This Privacy Policy applies only to information obtained through our Internet website.

I. Privacy Policy Summary

We use your contact information only to confirm your Internet order or to communicate with you regarding your assessment or any questions that you submit to us. We do not share our email list, mailing list or phone numbers with anyone else.

We use a secure ordering process to ensure the safety of your credit card and personal information. Online transactions are handled through scripting designed to submit orders with Secure Sockets Layer (SSL) encryption. When you enter your credit card number and personal information into the Acumen Biologics Order Form, it is transmitted across the Internet in an encrypted (scrambled) form, and then decoded once it gets to us. This provides a reliable and confidential ordering environment for your security. You can also order over the telephone at 1.801.949.9355 between 9 a.m. and 5 p.m. Mountain Standard Time or by mailing in an Order Form.

We at Acumen Biologics understand and are committed to upholding the privacy of your personally identifiable and personal health information. This is priority number one for us. III. Information Collection

Non-Personal Information

If you visit our Internet website, but you do not order the N.O.W. Profile, the only information we collect will be Non-Personal Information through the use of Cookies. Non-Personal Information is not traceable back to you, cannot be used to identify you and includes information such as the Internet web pages that you have viewed. A Cookie is a small text file that is stored on the hard drive of a computer used to view websites that is accessible only by the party or website that placed the Cookie on the hard drive. Cookies enable Acumen Biologics Corporation to track usage patterns and deliver customized content to users.

We collect these data on an anonymous, aggregate basis. The information we collect may include the time and length of your visit to our website, the pages you look at on our website and the website you visited just before coming to ours. We may also record the name of your Internet service provider. We use this information to measure site activity. We do not use information to track or record information about individuals. Generally, we delete this information after one year.

Personally Identifiable and Personal Health Information

If you choose to order the N.O.W. Profile or ask a question on our website, we require you to submit Personally Identifiable Information (i.e., information that can be traced back to you. Personally Identifiable Information includes information such as your name, address, telephone number and email address). Some of this information may identify you as a N.O.W. Profile client and allow you to access your account, which contains your private, personal and medical information.

If you order the N.O.W. Profile, we also may collect Personal Health Information (i.e., the combination of Personally Identifiable Information with known health characteristics). Personal Health Information includes the combination of your Personally Identifiable Information with information that you provide to us in your Medical History Questionnaire. You are responsible for ensuring the accuracy of the Personally Identifiable Information and Personal Health Information. Information about your medical history is used to aid in the analysis of your N.O.W. Profile results. Inaccurate information will affect the analysis you receive in your personalized report.

If you order the N.O.W. Profile, personal information and information that identifies you as a N.O.W. Profile client are mandatory because that information is needed to set up your N.O.W. Profile account. Other information we request is optional. If you have any questions about mandatory or optional information, please call us.

In accordance with the Children’s Online Privacy Protection Act (COPPA), Acumen Biologics does not actively market to children, and we never knowingly ask a child under 13 to divulge personal information.

IV. Information Usage

The information collected by Acumen Biologics will be used for the creation of your N.O.W. Profile account, aggregate data collection and demographic data, email communications and analysis of your N.O.W. Profile results. Users who provide information may receive email communications, information about their account and information about their N.O.W. Profile results.

Acumen Biologics will never make any personal identifiable individual information available to third parties for third-party marketing or other purposes, without your explicit agreement.

Users may delete their information from our database by sending us a written request. We will delete your information and send you confirmation that the information has been deleted.

Note Regarding State Law

For all of the above purposes, in cases where state law is more restrictive than federal law, we are required to and will follow the more restrictive state law. For example, some states only allow physicians, and not consumers, to order laboratory tests, and other states prohibit a laboratory from releasing test results directly to a patient.

VI. Linked Sites

For your convenience there are links to websites operated by companies/organizations. We do not control the content that appears on these sites and their content may change. These linked sites may have privacy policies that differ from Acumen Biologics’. If you visit one of these linked websites, we suggest that you review that website’s privacy policy.

VII. Problem Resolution

If problems arise, users may contact Acumen Biologics by phone, email or regular mail. We are committed to resolving disputes within 20 business days.

VIII. Data Storage and Security

When you complete the N.O.W. Profile Medical History Questionnaire, the information is sent directly to a secure database. The database resides in a protected facility with sophisticated security systems and controlled physical access. Access to all data is permitted ONLY to selected individuals. No general access is provided within or outside the organization. All pre-authorized individuals must provide a user ID and password match. Accessing your profile always requires your user ID and password, as well as an accession number that is provided when you receive your report. We recommend keeping your user ID, password and accession number (accession number provided in your N.O.W. Profile report) in a secure place, as this information allows you to access your personal information.

Acumen Biologics protects user information with the following security measures:
Acumen Biologics uses firewalls to protect information held in our servers.,br> Acumen Biologics utilizes Secure Sockets Layer (SSL) encryption in transmitting Personally Identifiable Information to our servers. To take advantage of encryption technology, you must have an Internet browser which supports 128-bit encryption.
Acumen Biologics requires all Acumen Biologics employees to abide by our Privacy Policy and be subject to disciplinary action if they violate it.
Acumen Biologics backs up our systems to protect the integrity of your Personally Identifiable and Personal Health Information.

Despite Acumen Biologics’ efforts to protect your Personally Identifiable Information and Personal Health Information, there is always some risk that an unauthorized third party may find a way around our security systems or that transmissions of your information over the Internet will be intercepted. If there is a breach, we will notify you.

IX. Changes To This Privacy Policy

If our policy on information collection or uses changes, Acumen Biologics will advise you by posting a change notice to this page. This page should be reviewed from time to time so that you are aware of any changes.
